General Form

Author or Agency. Title. City of Publication (state): Publisher; Year.

Some government agencies are buried within other larger agencies.  You need to list the hierarchy of the bureaucracy starting with the largest agency and narrowing down to the smallest agency that actually commissioned/ authored the documents.

When discussing national documents, you will need to indicate which country.  The names of countries can be abbreviated. A list of ISO abbreviations for countries is listed in Appendix 29.3 in the CSE Style Manual.
